Why wasn't a mechanism in place to prevent the disastrous effects of Deepwater Horizon Oil spill?

Yes, there was a failsafe valve shutoff for the oil pipe but in the explosion that triggered the oil leak it seems that they discovered it damaged the shutoff and at the time it was supposed to work it could not. They only had one emergency shutoff prepared so once that failed they had no secondary choice. I hope this helps.
