I need to make a form that can acess a timetable. How can I do this?

I need to make a form for my website (a href="http://www.aavg.org/)" rel="nofollow"http://www.aavg.org/)/a that has the ability to acess a list of flights. The form would have two drop down menus on it that would list airports. It would have to look something like this: a href="http://www.fly-twva.com/" rel="nofollow"http://www.fly-twva.com//a (look under the "SELECT A FLIGHT" section). Thanks!
